Student Absence Form

If your child is unwell, please keep them home from school until they are well enough to return. This allows them to rest and recover and also prevents illness spreading to other students and staff.

If your child is absent from school, it is important that you let the office know. Parents can:

• email the office at phoenix.ps@education.wa.edu.au
• call the office on 9414 2100
• Use Compass to send an absence message or respond to one sent from the school 

We use COMPASS to manage attendance at our school. The system automatically sends a text to your mobile if your child is absent from school. Please reply with the reason and we will update our records accordingly.  

Parents are asked to please use this form to request an absence for your child/children from school to participate in a holiday or other activities. This form should be submitted at least 4 weeks prior to the requested departure from school (except in extenuating circumstances).
